Helping You Manage It All
The role of non-medical case managers is to help people living with HIV to link to and stay in medical care. The non-medical case manager assigned to you will work with you to identify your needs and goals, help you link to resources, and resolve barriers to accessing medical, social, legal, financial, and other necessary services in the community. The goal is for you to be able to access the resources that you need to navigate the health care system and stay in medical care.
Once you have met your non-medical case management goals, you will graduate from the program. If you need non-medical case management assistance after your graduation, we can help you to re-engage services.
