Access Your Medical Records through the Patient Portal

Prism Health North Texas is pleased to announce easy and timely access to your medical records through the patient portal. The new features that are included are:

• Open Notes- all office visit notes and letters, signed by your provider.
• Lab reports, pathology reports, and imaging reports will be immediately available.
• Problems, medications, and allergies will be available in an easy-to-read format to share with other providers.
• Download your records by pressing ” the blue button.”
• Submit a request to modify or correct information in your record.
